This Privacy policy applies to all staff employed by Functional Physio SA over all clinic locations. This Privacy Policy does not extend to any personal information collected or held by Functional Physio SA in relation to its employees.

Functional Physio SA abides by the relevant National Privacy Principles of the Privacy Act 1988. The APP’s regulate the way personal information is handled, from collection, to use and disclosure, storage, accessibility and disposal. Functional Physio SA is also required to comply with the Spam Act 2003 (Cth) (Spam Act) and the Do Not Call Register Act 2006 (Cth) (Do Not Call Register Act). We are committed to protecting your privacy. Much of the information on this form is sensitive information. Sensitive information will not be used for direct marketing purposes. The information on this form is used for the purpose of providing us with the background as to your past and present medical details. The types of organizations to whom we usually disclose this information will be health care providers including our sports trainers and sports first aiders but may also be viewed by coaches, directors and officials. We may also have to disclose it to our professional advisers and insurer. If you do not provide us with any or all the personal information that we request, then you may not be able to play sport in any of our teams. Personal Information includes, but is not limited to, the following: Names, addresses and contact details including email address, mailing address, street address and/or telephone number; your age and/or date of birth; additional personal information that you provide to us, directly or indirectly, through your use of our Site, associated applications, associated social media platforms and/or accounts from which you permit us to collect information; any other personal information requested by us and/or provided by you or a third party (We may collect these types of personal information directly from you or from third parties); Private health care details and Medicare number (where available) for identification and claiming purposes; and Medical information including medical history, medications, allergies, adverse events, social history, family history and risk factors. Clinical notes of an individual’s symptoms, diagnosis and the treatment given. Specialist reports and test results, Appointment and billing details, Prescriptions and other pharmaceutical purchases, an individual’s healthcare identifier when it is collected to provide a health service. Any other personal information (such as information about an individual’s race, sexuality, religion, date of birth, gender) reasonably necessary to provide a health service.
Functional Physio SA will not collect personal information about you unless that information is necessary for one or more of our functions or activities.

Functional Physio SA will collect personal information only by lawful and fair means and not in an unreasonably intrusive manner.

Functional Physio SA may collect your Personal Information in a number of ways including, but not limited to: Directly from you, for example: the information you provide to Functional Physio SA in any Patient Information Sheet; During the course of providing clinical services to you; From your guardian or responsible person; and From any other involved healthcare specialists.

Functional Physio SA may store your Personal Information in various forms including: Paper records; Electronic format; As visual – x-rays, CT scans, videos and photographs; and as audio recordings. Functional Physio SA takes reasonable steps to maintain the security of your Personal Information and to protect it from unauthorised disclosures.

When Functional Physio SA collects personal information directly from you, we will take reasonable steps at or before the time of collection to ensure that you are aware of the purposes of collection, any other organisations (or types of organisations) to which we would usually disclose information of that kind, detail on how you access the information and how to contact us. The personal information you provide to Functional Physio SA will be collected for the primary purpose of providing clinical services and treatments to you; and for claims and payments. From time to time, Functional Physio SA will contact its Clients and non-Clients whose personal information has been collected for a range of reasons including: Appointment Reminders, Appointment Follow Ups, Marketing and promotional material about new products, services or special offers, Periodic Newsletters, Practice Updates, To provide you with information about the current and future benefits of being a Functional Physio SA client, Market research or surveys to improve our products and services. Functional Physio SA may disclose your Personal Information to third parties who are engaged by Functional Physio SA. These third parties are required to comply with this policy. Functional Physio SA may also disclose your Personal Information: To your authorised representatives or advisers, or otherwise when you ask Functional Physio SA to do so; To other Health Professionals involved in your care e.g. GP, Specialist, other Allied Health Professionals; and when there is a statutory requirement for Functional Physio SA to do so. Financial institutions for payment processing. Other times may include Government and other regulatory bodies such as Department of Veterans Affairs, Transport Accident bodies and Workers Compensation bodies, Your employer, where required and permissible in relation to a work-related injury, and Functional Physio SA’s contracted service providers, including: Information technology service providers; Printers and distributors of direct marketing materials; and External business advisors (such as auditors and lawyers).

In these circumstances, Functional Physio SA will notify you that there is a statutory requirement to disclose certain personal information, e.g. Insurance Companies, Health Fund agencies; and to Government and regulatory authorities as required or authorised by law.

We will not sell, distribute or lease your personal information to third parties unless we have your permission or are required by law to do so. We may use your personal information to send you promotional information about third parties which we think you may find interesting if you tell us that you wish this to happen.

We may occasionally hire other companies to provide services on our behalf, including but not limited to handling customer support enquiries, processing transactions or customer freight shipping. Those companies will be permitted to obtain only the personal information they need to deliver the service. Functional Physio SA takes reasonable steps to ensure that these organisations are bound by confidentiality and privacy obligations in relation to the protection of your personal information.
